Full time employees (64 hours+) and eligible part time employees (40 hours+)
Three tier plan with the highest level of coverage provided by using Partnership facilities, University Hospitals of Cleveland, and OHCP panel physicians. $10.00 co-pay for physician office visits and $20.00 co-pay for specialist office visit. There is also a $200.00 individual / $400.00 family annual deductible.
Two level plan with highest level of coverage provided by using Partnership and University Hospitals of Cleveland facilities. $15.00 co-pay for office visits and $20.00 co-pay for specialist office visits. There is also a $100.00 individual/ $200.00 family annual deductible.
All plans include behavioral health coverage, and one annual eye exam.
Prescription drug coverage is provided through Caremark
** If you are enrolling a child age 19-23 who is a full time college student you will need to provide a proof of student status when signing up for benefits and provide Human Resources with this information every quarter/semester.
Open access to any dentist-however participants may have a lower out of pocket expenses by using a panel dentist.
Coverage provided through St. Lukes Dental Associates. Participants must use a panel dentist. Offices are conveniently located throughout the greater Cleveland area.
1x pay provided at no charge. Employees may elect an additional 4x pay at their own expense.

Note: Employees in positions budgeted for 32 or more hours per pay period are eligible to earn PTO.
Employees earn PTO each pay period based on actual hours worked per pay period. PTO time will not show on your paycheck until you have completed 90 days of employment.
Example:
A new full time employee in an hourly position, normally working 80 hours per pay will earn 7.08 hours of PTO each pay period (184 hours per year).
PTO Cash-In
PTO may be cashed in twice a calendar year with a maximum of 120 hours. Hours are paid at $.90 on the dollar. (A maximum of 40 hours must remain in PTO bank following a cash-in)
PTO Donation-
Employees may donate accrued PTO time to another SJWS employee who has no PTO time for employees own illness and is on FMLA.
Provided at not cost to full time employees who have completed one year of service. Begins after 3 days of disability /illness or immediately for emergency hospitalizations/out patient surgery.
The plan replaces 60% of the employee’s base pay for up to 5 months.
Coverage to replace 60% of base pay after 5 months of disability.
Administered through Fidelity Investments. Employees may contribute between 1% and 15% of pay up to $10,000 annually on a pre-tax basis. The Hospital will match 50% of the first 4% of contributions. Employees are 100% vested in the Hospital matching portion after 3 years of service. (Note: employees must work at least 1,000 hours in the year to earn one year of service towards vesting)
Administered through Fidelity Investments. The Hospital will contribute to this account (at the beginning of each year) a percentage of the employee’s annual base pay. The percentage of pay contributed is based on years of service(2.75% for less than 5 years of service up to 6% of pay for 20 or more years of service). Employees are 100% vested in this account after 5 years of service. (Note: employees must work at least 1,000 hours in the year to earn one year of service toward vesting)
FULL TIME/ PART TIME : After one year of service employees are eligible for full time $1,500.00, part time $750.00 reimbursement per calendar year on tuition when enrolled in undergraduate courses given by recognized schools, colleges and universities in which grades are earned and employee earns a grade of “C” or better.
