Community Outreach Services

The Department of Community Outreach

The Community Outreach Department was established in 1998 to promote health and wellness in the communities we serve. The Department is funded by St. John Medical Center, grant support, proceeds from The Festival of the Arts, Hospital Auxiliary donations, Celebrate Westlake and private donors.

St. John Medical Center’s Community Outreach Program promotes “Preventative Healthcare” through the following:

  • Physician and health talks
  • Health screenings
  • Support groups
  • Immunizations
  • Healthcare assistance for the underserved
  • Babysitting and parenting classes
  • Healthcare Access social worker
  • Wellness Ministry
  • Breast Health Program
  • Mobile Medicine Care Program

This Department works in tandem with community groups and organizations, local schools, senior centers and assisted living facilities.  Demographically, our program extends throughout western Cuyahoga and eastern Lorain counties.

Mobile Medical Care

The Mobile Medical Care Program, which was founded in 1986, continues the mission of the Hospital by providing traditional “doctor house calls” to the homebound elderly, age 65 or older who have traditional Medicare. Program assistance is given to those who may be living at home or in an assisted living facility.  A major program component is continuity of care to those patients who may have no regular physician and who are in need of ongoing care.

The program offers the following services:

  • Home-delivered medical services by a physician and nurse
  • Geriatric assessment, complete history and physical exam, patient education and follow-up care
  • Basic blood tests, EKG, oximetry and blood pressure monitoring
  • Coordinate care with home health agencies, dentists, podiatrists, prescription refills and alternative living arrangements on an as needed basis
  • Preventative care such as flu/pneumonia vaccine and wound care
  • Family support and assists in end stage or terminal care at home

Prescription Assistance Program

Recognizing the need to assist those who may be experiencing a recent financial setback, St. John Medical Center’s Community Outreach Department has developed a special program for patients who cannot afford necessary medications.  Its intent is a one-time use for patients who are discharged and have no other resources. After consultation by St. John Medical Center Discharge Planners, a two-week supply of medications is provided.  In addition, a referral is made to our Healthcare Access Social Worker who then assists patients with enrollment in prescription drug programs for their longer-term needs.

Breast Health Program

Whether you have a question about prevention, or you or a loved one is dealing with a diagnosis of breast cancer, our Breast Health Coordinator can help.  She is a nurse with Advanced Certification in Breast Health and can answer and/or provide referral information for any breast health related question you may have.  She is a tremendous support person throughout a woman’s journey with Breast Cancer.  In addition, she can be a valuable liaison between you and your healthcare provider.  She provides pre- and post-operative patient care and teaching as well as community education and support to patients, families and the broader community.  Her services are free, as is our monthly Breast Cancer Support Group.

Health Access

The Healthcare Access Program is designed as a resource for the uninsured and underinsured in meeting their healthcare needs.  A Licensed Social Worker serves as our Healthcare Access Case Manager by evaluating clients’ needs and assisting them with long-term resources, physician services, laboratory and radiology access and indigent drug programs.  The Case Manager is also available to assess needs of those who are having a difficult time in paying for medications, transportation, home healthcare services, or who are seeking alternatives to a nursing home placement.  This program was originally funded through a grant, but is currently supported by the Hospital. Program referrals can be made by anyone including community agencies, churches and physician offices.

The Wellness Ministry

Our Wellness Ministry Program is designed to work with churches. It brings rich and supportive relationships to a congregation with the skills and expertise of healthcare professionals and hospital resources.  This program includes churches of all denominations and is based on a volunteer model of parish nursing.

This program truly fosters wellness and healing in a holistic manner, encouraging preventative healthcare and associated education.   The Hospital supplies the churches with orientation, basic starting equipment, literature, health talks and screenings.

In addition, we provide our congregations with the Healthcare Access Assistance Program.  This is a valuable resource for the uninsured as well as the underinsured.

Special Events

The St. John Medical Center campus is the site of many annual community events, including the Festival of the Arts, a three-day creative presentation featuring artists from across the Midwest as well as Celebrate Westlake, the city's annual walk/run in support of area charities.
